The DentalScouts Records pilot form collects your email address, role (patient or dental practice), and any practice name or city you choose to provide. We use this information to contact you about the pilot and understand interest. We do not request, receive, or process dental records through these pages.

Please do not send X-rays, clinical notes, patient names or medical information through these forms. Pilot interest is stored in our Cloudflare infrastructure. Contact hello@dentalscouts.com to request deletion of your early-access details.
